OPT OUT SURVEY FORM

In order to best educate your child, we are asking all students to complete an online survey at school. This survey will give us some insight into your child's experience at Lincoln and will help us make any needed improvements. If you DO NOT want your child to complete this survey please print out this document, fill in your information, and return it to school by Monday, 3/1 ( A day) or Thursday, 3/4 (B day).
